New Patient Registration

You will be required to call into the practice bringing with you your NHS medical card if you have one. If you do not have your medical card you will need to complete a registration form.

You will also need to provide proof of residency which has your full name and address; please note this is for verification only - we do not keep these copies.

A new patient questionnaire for each member of the family wishing to register will also require completion. A registration pack will be given to you, which will include a copy of the practice booklet and details of the out-of-hour’s service.

A health screening is included in the registration process. This is undertaken by a nurse and should take about 20 minutes including a check of your blood pressure, weight, height and urine. You will be asked to provide relevant information about your health background.

Changes to Personal Details

Please help us to keep your records accurate by informing us of any change to name, address or telephone number.
